Pharmacological Action

Aprepitant is an antiemetic that acts as a neurokinin-1 (NK1) receptor antagonist.
It prevents nausea and vomiting associated with chemotherapy by blocking central pathways involved in the vomiting reflex.

Indications

Used for:

• prevention of acute and delayed chemotherapy-induced nausea and vomiting (CINV)
• prevention of postoperative nausea and vomiting (in selected cases)

Dosage and Administration

Standard 3-day regimen:

• Day 1: 125 mg 1 hour before chemotherapy
• Days 2–3: 80 mg once daily

Often used in combination with other antiemetics.

Contraindications

• hypersensitivity to aprepitant
• concomitant use with certain drugs (e.g., pimozide, terfenadine)
• severe hepatic impairment

Warnings and Precautions

• Potential drug interactions (CYP3A4 pathway)
• Use cautiously in liver disease
• May reduce effectiveness of hormonal contraceptives
