Leucovorin (folinic acid) and autism: what the research shows

Leucovorin is a prescription folate medicine studied in children with autism and cerebral folate deficiency. What clinical trials show, who may respond, and what to discuss with your physician.

TL;DR

Leucovorin (folinic acid) is a prescription folate medicine studied in children with autism spectrum disorder (ASD), especially those with cerebral folate deficiency (CFD) - impaired folate transport into the brain. Randomized trials show verbal communication improvements with medium-to-large effect sizes, particularly in children with folate receptor autoantibodies (FRAAs). It is prescription-only: testing, dosing (typically 0.5-2.5 mg/kg/day, max 50 mg/day) and monitoring are set by a physician. Four original leucovorin products are available at Medivitus Pharma with worldwide shipping - compared below.

What is leucovorin (folinic acid)?

Leucovorin (folinic acid, calcium folinate) is a biologically active form of folate - vitamin B9. Unlike ordinary folic acid, it enters the folate cycle without the DHFR conversion step and can reach the brain through alternative carriers when the main transporter is blocked. It has been used for decades in oncology (protecting healthy cells from chemotherapy) and for folate deficiency states.

Why folate matters for the brain: cerebral folate deficiency

The brain uses the 5-MTHF form of folate, carried across the blood-brain barrier by the folate receptor alpha (FR-alpha). When this receptor is blocked by autoantibodies (FRAAs) or impaired by mitochondrial dysfunction, brain folate drops despite normal blood levels - this is cerebral folate deficiency (CFD).

A 2021 systematic review and meta-analysis (Frye et al., Journal of Personalized Medicine) reported: about 44% of children with CFD have an autism diagnosis; signs of CFD are found in around 38% of children with ASD; folate receptor autoantibodies were detected in up to 71% of studied children with ASD.

What the clinical trials show

Verbal communication (Frye et al., 2018, Molecular Psychiatry):

 randomized double-blind placebo-controlled trial, 48 children with ASD and language impairment. 12 weeks of high-dose folinic acid (2 mg/kg/day, max 50 mg/day) significantly improved verbal communication (Cohen's d = 0.70); in FRAA-positive children d = 0.91.

Systematic review (2021):

leucovorin was associated with improvement in overall ASD symptoms (67% of cases), irritability (58%), ataxia (88%), pyramidal signs (76%), movement disorders (47%) and epilepsy (75%) in children with comorbid CFD.

Combination therapy:

folinic acid + methylcobalamin (B12) trials showed improvements in communication and folate metabolism markers.

Safety:

side effects in controlled trials were generally mild and comparable to placebo.

Regulatory note: in 2025 the US FDA initiated a label expansion for leucovorin calcium for cerebral folate deficiency - a condition with developmental delays and autistic features. The evidence base is growing but still developing.

The four leucovorin products in our catalog - what is the difference

All four contain folinic acid salts (calcium folinate / leucovorin calcium) - the same active molecule. The differences are the form of the salt, the dosage per tablet, pack size and the manufacturer. Here they are:

1. Lederfolin 7.5 mg (Calcium levofolinate) - Pfizer, Italy

Composition: calcium levofolinate 7.5 mg - the pure active L-isomer of folinic acid. Pack: 10 tablets. Manufacturer: Pfizer (Italy).

The levofolinate form matters: only the L-isomer is biologically active, so the dose of 7.5 mg levofolinate is roughly equivalent to a higher dose of a racemic (mixed-isomer) product. This is the flagship European original - used both in oncology rescue protocols and in folate metabolism support. Prescription-only.

2. Lederfoline 5/15/25 mg (Calcium folinate) - Pfizer, France

Composition: calcium folinate in three strengths - 5 mg, 15 mg, 25 mg per tablet. Pack: 30 tablets. Manufacturer: Pfizer (France).

Same molecule as Lederfolin, but a racemic calcium folinate with a wider dose range in one product line - convenient when the physician titrates the dose (e.g., in weight-based schemes for children or in long-term maintenance). Prescription-only.

3. Leuconamo-15 (Leucovorin Calcium 15 mg)

Composition: leucovorin calcium 15 mg per tablet. Pack: 20 tablets (2 x 10). Manufacturer: Shivnaam Pharmaceuticals / Tradelink LLP (India).

A 15 mg leucovorin tablet meeting the USP quality standard, with product authentication on the pack. Strength-wise it is similar to the 15 mg dose of Lederfoline - the difference is the manufacturer and price. Prescription-only.

4. Lederfolat 5 mg (Calcium folinate) - 100 tablets

Composition: calcium folinate 5 mg per tablet. Pack: 100 tablets - the largest pack in the lineup, designed for long-term courses. Prescription-only.

The 5 mg strength with a 100-tablet pack suits long low-dose schemes, when the physician maintains a steady background dose rather than high pulsed doses.

Quick comparison

Want the pure active isomer (levofolinate):

 Lederfolin 7.5 mg by Pfizer.

Want dose flexibility (3 strengths in one line):

 Lederfoline 5/15/25 mg by Pfizer.

Want a proven 15 mg dose at a friendly price:

 Leuconamo-15.

Want long-term low-dose course (100 tabs):

 Lederfolat 5 mg.

All four are prescription-only medicines: the choice of product, dose and course is made by the physician based on the indication, body weight and monitoring results.

Who may be a candidate for leucovorin therapy

Leucovorin is not a universal autism treatment. The evidence is strongest for children with measurable folate-transport problems.

Physicians typically consider:

testing for folate receptor autoantibodies (FRAA) in blood;

clinical picture: language impairment, regression, seizures or movement problems together with ASD features;

response monitoring over a 12-week trial with weight-based dosing.

Behavioral interventions (ABA, speech therapy) remain the foundation of support - folinic acid is studied as an adjunct, not a replacement.

Safety and side effects

Across placebo-controlled trials, side effects were generally mild and comparable between treatment and placebo groups - occasionally agitation, insomnia or headache in a minority of children. Any changes in behaviour during the course should be discussed with the treating physician.

FAQ

Is leucovorin the same as folic acid?

No. Folic acid is a synthetic oxidized form that requires conversion in the body. Folinic acid is already biologically active and matters in folate transport problems.

Does leucovorin cure autism?

No. There is no cure for autism. Trials show improvement in specific functions - primarily verbal communication and attention - in a subgroup of children. Outcomes vary from child to child.

What is the difference between levofolinate and folinate?

Commercial folinic acid is usually a racemic mix of two isomers; only the L-isomer (levofolinate) is biologically active. Lederfolin contains pure calcium levofolinate, while Lederfoline/Leuconamo/Lederfolat contain racemic calcium folinate - which is why their doses are numerically higher for the same effect.

How long until an effect appears?

In trials, changes were assessed after 12 weeks of treatment. The full response is typically evaluated after the course.

Which tests should be discussed with the doctor?

Folate receptor autoantibodies (blocking and binding FRAA), and in some cases CSF analysis (invasive, only by neurologist's decision), plus general metabolic screening.
